Load Securing Near Miss

What Happened:

On 10th January 2014 a trailer loaded with Wire Rod in Coil left the Scunthorpe Rod Mill trailer park unsecured. The haulier was contacted and the load stopped, inspected and secured before proceeding to the delivery point.

Findings:

  • Collecting driver had not visited the site before
  • Driver did not follow the reporting procedure on site and was not fully briefed on site requirements and booking in process by the traction provider.
  • Traction provider had not informed the Trailer Operator that an Agency driver was being used, which is in breach of the SLA Agreement.
  • Driver had been issued with the collecting trailer number prior to arrival at site enabling him to go to the trailer directly.

Recommendations:

  • Brief first time drivers of the site procedures prior to arrival at site.
  • Hauliers are NOT to issue trailer numbers to collecting drivers.
  • Hauliers and Operators to regularly Audit Traction Providers to ensure sufficient training, safety, communication and driver standards are maintained.
  • Site access restrictions placed on Traction Provider with immediate effect 0600- 2200 Monday – Friday Only; all loads to be checked by Safety Inspectors before being allowed to leave site.
