Unexpected Door Movement - Minor Injury

What Happened:

Whilst strapping load on wagon the left hand door came adrift from the catch. The force of the the wind on the day caused the the door to work free from the door retaining arm and swing open and hit the IP in the face causing a minor laceration above the left eye. The IP was given First Aid treatment and later discharged. The IP returned to work with no further issues.

Unexpected Door movement

Findings:

  • High winds had caused the door to rattle / vibrate and the restraining arm had slipped down.
  • IP was wearing full PPE

Recomendations:

  • Check retaining arms for correct tension
  • Report any defects to retaining arms as soon as possible
  • Ensure adverse weather (strong winds) is considered in all risk assessments
  • Be extra vigilant during strong winds
  • Consider the use of additional devices to doors during strongs winds.
